Our Transitional and Palliative Care services provide an added layer of support to help you manage the challenges that accompany serious illness. These are not hospice or end-of-life services. Our professional, caring team works alongside your existing team of doctors and other care providers. We’re here to help so you can focus on what’s most important to you.
Transitional Care
Our home-based Transitional Care service provides an extra layer of guidance and emotional support. A Transitional Care Liaison meets with you to identify your needs and connect you with community resources that offer additional support. With a focus on improving planning and communication among your care team (you, your family, and medical providers), our Transitional Care services are tailored to enhance your overall wellbeing.
Palliative Care
Simply put, palliative care provides relief from pain and other symptoms and stress of serious illness. Our home-based Palliative Care service provides a team approach to your care that includes you and your family, your current doctor and care providers, and our Palliative Care team. We will never assume to know what kind of care you want. Instead, we’ll talk in depth with you and your family to reflect on your goals, ensuring the care you receive matches your needs and values.
Our Transitional and Palliative Care programs are fluid and flexible based on your care goals at the time. You can move between the services based on your condition and symptoms getting better or worse. The idea is to stay on top of it, and give you the best possible support for what you are experiencing.
