Bequest in a Will or Living Trust
The most common planned gift is a bequest in a will or living trust. If you choose this option, below is some suggested language.
“I give and bequeath to the trustees of Hospice of Santa Cruz County currently located at 940 Disc Dr, Scotts Valley, CA 95066, ________ % of my total estate or $ _________________ or other property.” (US Federal ID 94-2497618).”

Unrestricted Gift
For an unrestricted gift that allows Hospice of Santa Cruz County to determine how to use the funds based on the most pressing needs, use the following language for your will and/or trust:
“I give to Hospice of Santa Cruz County, a nonprofit corporation currently located at 940 Disc Dr, Scotts Valley, CA 95066, or its successor there to, _______________ [written amount or percentage of the estate or description of property] for its unrestricted charitable use and purpose.”
A Gift for a Particular Purpose
To designate your provision for a particular purpose:
“I give to Hospice of Santa Cruz County, a nonprofit corporation currently located at 940 Disc Dr, Scotts Valley, CA 95066, or its successor thereto, ________________ [written amount or percentage of the estate or description of property]. This gift should be used for [state purpose] if and so long as Hospice of Santa Cruz County determines that this need exists. If Hospice of Santa Cruz County shall determine at the outset or at a later time that this need does not exist, or no longer exists, or for some reason it is not possible (or prudent) to administer my gift as originally intended, then Hospice of Santa Cruz County may, in its sole and uncontrolled discretion, direct the use of my bequest for a purpose related as closely as possible to that purpose stated above.”
