When you or someone you love has a serious illness, it can affect your life in countless ways. Normal, daily routines have changed. Sometimes it feels as if you need to learn a whole new language to understand what’s happening. It can be confusing to know where to turn for help. We understand and are here for you.
The Transitions program provides support by helping ease concerns and identifying community resources to assist you with healthcare issues. A caring staff of medical social workers can provide support to you in the comfort of your own home by:
- Helping you and your family better understand the disease process, treatment options and your goals for care
- Assisting with important documents including Advance Healthcare Directives so your healthcare wishes are clearly communicated
- Helping to create a care giving plan to address your changing healthcare needs
- Identifying community resources to assist with financial concerns, emotional needs and practical support during this time of uncertainty
How Transitions Works:
Transitions is available to anyone with a serious illness or those wishing to explore future healthcare decisions. Transitions’ professional, caring staff can help you, your family and caregivers create systems to help you feel more confident as you navigate through challenging healthcare decisions.
A fee for services is based on a sliding scale. Call 430-3058 and let us share how we can help.
