Provide skilled professional end of life care to patient/family/caregivers including expertise in pain and symptom control. Work collaboratively with patients/family, the physician and interdisciplinary team to identify physical, psychological, social and spiritual needs and develop an individualized Plan of Care. Assist in facilitating patient autonomy, relief of suffering and death with dignity for patients facing end of life issues. Provide support and education for families based on the hospice philosophy of care. Be efficient and organized yet skilled at sensitivity and compassion to help ensure all patient and family care needs are being met.

Provide support and education for families based on the hospice philosophy of care. Be efficient and organized yet skilled at sensitivity and compassion to help ensure all patient and family care needs are being met. Prefer technical experience with ports, IV’s, TPN and 2-3 years RN experience in hospice, oncology, medsurg, ICU or homecare. Previous experience working with end of life is desirable. Bilingual in English/Spanish encouraged but not required. Require current California RN License, valid state driver’s license and liability insurance.
